Who Is a Candidate for Ketamine Therapy in Missouri?

Who it is usually considered for

Off-label racemic ketamine is sometimes considered in similar situations when a licensed prescriber judges the evidence and the individual history carefully. That is a clinical judgment, not a consumer product choice.

What “treatment-resistant” means in practice

Insurers and clinics both use the phrase, but they do not always mean the same thing. A clinic may be willing to discuss treatment after two failed antidepressant trials; a payer may still deny coverage until each trial is documented by name, dose, duration and reason for stopping. Bring that documentation to the first assessment if you have it.

Contraindications and cautions a clinician will screen for

None of these bullets is a self-exclusion checklist. Some are hard stops; others are reasons for extra monitoring, a different modality, or waiting. Only the assessing clinician can place you on that map.

What the assessment usually involves

  1. A full psychiatric history, including prior medicines, therapies, hospitalisations and suicidality.
  2. Medical history and often a blood-pressure check; some clinics order labs or an ECG.
  3. A conversation about goals, logistics (rides home, time off work), and cost or insurance.
  4. For Spravato, enrolment in the REMS programme and confirmation the site is certified.
  5. Informed consent that covers dissociation, blood-pressure rise, driving rules and uncertainty of benefit.

When it is not the right option

Ketamine-class treatment is not a first-line antidepressant, not a substitute for emergency care, and not a good fit when the primary problem is untreated mania, unstable medical illness, or a setting that cannot monitor you safely. Who may assess and prescribe in Missouri

Missouri physicians may prescribe and administer ketamine without restriction. An advanced practice registered nurse may be delegated authority to prescribe it under a written collaborative practice arrangement with a physician, but that arrangement cannot delegate authority to give a Schedule III to V medicine for the purpose of sedation or anaesthesia.
