Ketamine and Bipolar Disorder in Missouri
The evidence that ketamine helps bipolar depression is genuinely thinner than the evidence for unipolar treatment-resistant depression. That is the first fact on this page, not a footnote — because a clinic selling infusions has no commercial incentive to lead with uncertainty or switch risk.
Direct answer
Evidence that ketamine or esketamine treats bipolar depression is more limited than for unipolar major depression: small randomised trials and open-label series suggest short-term antidepressant effects in some patients, but sample sizes are small, follow-up is short, and no ketamine product is FDA-approved specifically for bipolar disorder. as of 1 Apr 2020 · Journal of Affective Disorders / PubMed (source 1, opens in a new tab)Do not read this as a treatment recommendation. A bipolar diagnosis changes the assessment and the risk profile; only a treating clinician can decide.
Only esketamine (Spravato) is FDA-approved to treat depression. Racemic ketamine given by IV, injection, or under the tongue has no FDA approval for any psychiatric condition and is used off-label — that is, an approved drug used for an unapproved purpose. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 2, opens in a new tab)
The FDA approved Spravato (esketamine) nasal spray on 5 March 2019, for use with an oral antidepressant, for adults with treatment-resistant depression. as of 5 Mar 2019 · U.S. Food and Drug Administration (source 3, opens in a new tab)
If a page tells you ketamine is “proven for bipolar” without naming a trial, sample size and date, treat that as marketing. Prefer “a trial of N participants found…” with a link.
What the studies actually show (and do not)

Typical limitations in this literature: small samples, short follow-up, mixed bipolar I and II populations, concurrent medicines that differ between studies, and outcome measures that capture days rather than months. Those limitations are why this page does not publish a success rate.
Randomised trials and systematic reviews report that a single sub-anaesthetic intravenous infusion of racemic ketamine can reduce depressive symptoms within hours in some adults with major depression or treatment-resistant depression, but the effect often fades within days to a week without a maintenance strategy, and racemic ketamine has no FDA psychiatric indication. as of 1 Apr 2019 · American Journal of Psychiatry / PubMed (source 2, opens in a new tab)This is research evidence, not an FDA approval. Off-label use remains lawful medical practice when a licensed clinician judges it appropriate.
Mania and hypomania switch risk
Because ketamine-class medicines can elevate mood rapidly, people with bipolar disorder carry a recognised risk of switching into hypomania or mania; responsible protocols usually require an established mood-stabiliser regimen, close observation after sessions, and a plan for what to do if activation or reduced need for sleep appears. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 2, opens in a new tab)The Spravato label focuses on dissociation, sedation, abuse potential and suicidal thoughts rather than a dedicated mania-switch section; the clinical risk is also described in the bipolar-depression literature cited on this page.
Spravato carries a boxed warning for sedation, dissociation, respiratory depression, abuse and misuse, and suicidal thoughts and behaviours — the same class of risks that justify the REMS clinic-only distribution system. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 6, opens in a new tab)
Warning signs worth agreeing on with your clinician in advance: sharply reduced need for sleep, pressured speech, grandiose plans, agitation, or a family member saying you are “not yourself” in the activated direction. Depression lifting is the goal; activation without sleep is not a success.
Why a bipolar diagnosis changes the assessment
- Mood-stabiliser status and adherence become part of the safety plan, not an afterthought.
- Prior switches on antidepressants inform how aggressive any rapid-acting agent should be.
- Family history of bipolar disorder matters even if your chart currently says unipolar depression.
- Substance-use history interacts with a medicine that itself has misuse potential.
Spravato is contraindicated in people with aneurysmal vascular disease, arteriovenous malformation, or a history of intracerebral haemorrhage, and in anyone with a known hypersensitivity to esketamine, ketamine, or excipients — and a careful medical history is required before any ketamine-class treatment because blood pressure commonly rises after a dose. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 3, opens in a new tab)
What a responsible clinic will ask before treating
- Full mood history, including manic or hypomanic episodes you may not have labelled that way at the time.
- Current and past mood stabilisers, antipsychotics and antidepressants — with doses and responses.
- Hospitalisations, psychotic features and mixed states.
- Who will observe you after sessions and how sleep will be tracked between visits.
- Whether Spravato’s labelled indications and REMS logistics fit better than off-label racemic ketamine — or whether neither is appropriate yet.

Who may prescribe in Missouri
Missouri physicians may prescribe and administer ketamine without restriction. An advanced practice registered nurse may be delegated authority to prescribe it under a written collaborative practice arrangement with a physician, but that arrangement cannot delegate authority to give a Schedule III to V medicine for the purpose of sedation or anaesthesia. A physician assistant with controlled-substance certification may be delegated similar prescribing authority. as of 28 Aug 2023 · Missouri Revisor of Statutes (source 6, opens in a new tab)Whether the sedation restriction reaches low-dose ketamine infusions for depression — as opposed to anaesthesia — is genuinely unsettled in Missouri. No board guidance, attorney general opinion or court decision resolves it, so we do not state an answer either way. It is a fair question to ask a clinic about who will be in the room.
Psychiatric complexity is a reason to prefer clinicians who regularly manage bipolar disorder, not only elective infusion clinics. Verify licences here:

Frequently asked questions
Is ketamine FDA-approved for bipolar disorder?
No. Spravato is FDA-approved for treatment-resistant depression and for major depressive disorder with acute suicidal ideation or behaviour in adults, under labelled conditions — not specifically as a bipolar-disorder indication.
Can ketamine cause mania?
Rapid mood elevation is possible. People with bipolar disorder need protocols that watch for hypomania or mania, usually alongside a mood stabiliser, rather than depression-only monitoring.
Who can prescribe ketamine for mood disorders in Missouri?
Scope depends on licence type and supervision rules under state law — see the scope section on this page and verify any clinician through the state board lookup.
