Ketamine Side Effects, Safety and Risks in Missouri
Ketamine-class treatments can help some people with depression. They are also powerful dissociative anaesthetics with a real side-effect profile, a controlled-substance schedule, and monitoring rules that exist for a reason. This page is the plain version of those risks.
Common short-term effects
In treatment-resistant depression studies the most common side effects of Spravato were dissociation, dizziness, nausea, sedation, vertigo, reduced sense of touch, anxiety, lethargy, increased blood pressure, vomiting, feeling drunk, and headache. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 1, opens in a new tab)The label lists a different set for the major-depressive-disorder-with-acute-suicidality indication. The two lists should not be conflated.
Spravato carries a boxed warning for sedation, dissociation, respiratory depression, abuse and misuse, and suicidal thoughts and behaviours — the same class of risks that justify the REMS clinic-only distribution system. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 1, opens in a new tab)
During a session, blood pressure often rises for a while and then settles. Clinics check it for that reason. Nausea is common enough that many sites offer anti-nausea medicine. Tell staff early if you feel panicked, over-sedated, or like the room is too much — the dose rate can often be adjusted.
Dissociation — what people usually mean
Dissociation is a sense of detachment from your body, thoughts or surroundings. It can feel dreamlike, floaty, or frankly unpleasant. It is expected with these medicines at psychiatric doses, it is temporary for most people, and it is one of the reasons Spravato may only be given under observation.
A healthcare provider must monitor the patient for at least two hours after every Spravato dose before deciding they are ready to leave. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 2, opens in a new tab)
Bladder and urinary effects
Long-term, high-frequency recreational ketamine use is associated with ketamine-induced ulcerative cystitis — painful bladder, frequency, urgency and, in severe cases, irreversible damage — and while psychiatric clinic doses are far lower, urinary symptoms should be reported early rather than ignored. as of 1 Jan 2023 · StatPearls / NCBI Bookshelf (source 1, opens in a new tab)Most cystitis case series describe chronic recreational exposure, not a standard six-infusion clinic course. Absence of a published clinic-dose incidence figure is not proof of zero risk.
If you develop new urinary pain, blood in the urine, or a sudden need to void constantly during a course of treatment, stop and call the clinic rather than waiting for the next booster. Clinics that never mention this risk are not doing you a favour.
Dependence, tolerance and withdrawal
Ketamine is a Schedule III controlled substance with accepted medical use and a recognised potential for psychological dependence, tolerance with repeated high-dose exposure, and a withdrawal picture that can include craving, sleep disturbance and mood changes after heavy non-medical use. as of 18 Jul 2026 · U.S. Drug Enforcement Administration (source 2, opens in a new tab)Schedule status describes abuse potential relative to other controlled drugs; it is not a prediction that a monitored clinic course will produce dependence.
Ketamine and its mirror-image form esketamine are Schedule III controlled substances under federal law, which means they have accepted medical uses but can be misused. as of 18 Jul 2026 · U.S. Drug Enforcement Administration (source 3, opens in a new tab)Esketamine has no separate DEA listing; it is covered as an enantiomer of ketamine, and the Spravato label carries the CIII designation.
A monitored clinic course is not the same exposure pattern as daily recreational use. Still, ask any programme that pushes frequent at-home dosing without review how they watch for escalating use — and walk away if they cannot answer.
Half-life and how long effects last
After intravenous dosing, ketamine's elimination half-life in adults is typically about two to three hours, with psychoactive effects that peak during the infusion and usually settle over the following hours — which is why clinics plan observation time and why next-day driving rules still apply after esketamine. as of 1 Jan 2022 · U.S. Food and Drug Administration (source 3, opens in a new tab)
After a Spravato dose you must not drive or operate machinery until the next day, following a restful night's sleep, so you need to arrange a ride home from every session. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 6, opens in a new tab)
Feeling “mostly fine” an hour after a session is not permission to drive. The formal rule for Spravato is next-day driving only after restful sleep; many IV clinics apply the same caution.
Ketamine on a drug test
Standard workplace urine drug screens do not routinely include ketamine; detection usually requires a specific assay, and published detection windows after use are often on the order of a few days in urine depending on dose, frequency, metabolism and the laboratory method. as of 18 Jul 2026 · StatPearls / NCBI Bookshelf (source 3, opens in a new tab)Employers, courts and sports bodies may order expanded panels. If a drug test is pending, disclose prescribed treatment to the medical review officer rather than relying on a generalisation.
At-home and microdosing risk
At-home and microdosing ketamine programmes remove the on-site monitoring that clinic protocols use to catch blood-pressure spikes, severe dissociation, sedation and diversion — so the convenience trade-off is a real safety gap, not a minor difference in setting. as of 30 Apr 2025 · U.S. Food and Drug Administration (source 1, opens in a new tab)Spravato cannot be taken at home under the REMS. Compounded at-home racemic products are a different regulatory pathway and are not REMS-certified substitutes.
Missouri does not specifically require an in-person examination before prescribing a controlled substance, but a physician must first establish a proper relationship with you, including a medical history and an examination good enough to establish the diagnosis. A prescription cannot rest on a phone call alone without an existing relationship, or on an online questionnaire. as of 28 Aug 2025 · Missouri Revisor of Statutes (source 7, opens in a new tab)Federal DEA telemedicine flexibilities run separately and expire at the end of 2026.
Microdosing content online often treats ketamine like a productivity supplement. That framing erases respiratory depression, blood-pressure spikes, dependence risk and diversion. If a seller will not discuss monitoring, that is the answer.
Missouri Prescription Drug Monitoring Program (Missouri PDMP) in Missouri
Prescription monitoring is three different questions — what must be reported, when a prescriber must check, and how that applies to ketamine — not one slogan.
What must be reported
Missouri's statewide monitoring programme was created in 2021 and went live on 13 December 2023. Pharmacies and dispensing practitioners must report every Schedule II, III and IV controlled substance they dispense, in real time since 1 January 2024. Schedule V is not reportable in Missouri. as of 1 Jan 2024 · Missouri Revisor of Statutes (source 7, opens in a new tab)
When a prescriber must check
Missouri law does not require a prescriber to check the monitoring programme before writing a prescription — the programme's own guidance says so directly. The one exception comes from federal law: a prescriber must check before prescribing a Schedule II to IV controlled substance to a MO HealthNet patient. as of 13 Dec 2023 · Missouri Prescription Drug Monitoring Program (source 7, opens in a new tab)
Does this apply to ketamine?
Partly. Ketamine is Schedule III, so if a pharmacy dispenses take-home sublingual ketamine to you it is reported. Ketamine and Spravato given inside a clinic are generally not reported, because Missouri specifically exempts a practitioner who administers a medicine from the reporting duty. And because Missouri does not mandate prescriber checks, the one situation where a prescriber must look you up before prescribing ketamine is if you are a MO HealthNet patient. as of 1 Nov 2023 · Missouri Prescription Drug Monitoring Program (source 7, opens in a new tab)

Frequently asked questions
What are the most common ketamine side effects?
Dissociation, dizziness, nausea, sedation, raised blood pressure and headache are among the effects most often reported with esketamine; racemic clinic infusions produce a similar short-term profile.
How long does ketamine stay in your system?
Elimination half-life after IV dosing is typically about two to three hours, but residual impairment can last longer — Spravato patients must not drive until the next day after a restful sleep.
Does ketamine show up on a drug test?
Not on most standard workplace panels. Detection usually needs a specific assay; windows vary by dose and lab method.
Does Missouri track ketamine prescriptions?
Yes through Missouri Prescription Drug Monitoring Program (Missouri PDMP), with important differences between a written prescription, clinic administration, and compounded home products — see the PDMP section on this page.
